He is survived by: his wife Jessica Nicole Smith; his parents, Evans Howard Smith and Senia Diane Smith; his brother Stephen Howard Smith; his sister-in-law Kimberly Smith; his nephews, Jacob and Nathan Smith; his sister Lisa Shead (Carey Shead); his nephews, Tony Edge and Shawn Edge; his mother-in-law Kimberly Bodenhamer; his stepfather-in-law Doug Bodenhamer; his father-in-law Roger Jones; his stepmother-in-law Rachel Jones; his siblings-in-law, Hayden Jones, Alyssa Tyler and Nic Tyler; and his nephews, Bryceson Tyler and Kayson Tyler.

In lieu of flowers, the family is requesting donations to be made towards Maddie's college fund.
