He was predeceased by: his parents, William Avery and Agnes Lorena Lewis Slayton; his brother Fred Slayton; his aunt Mary Alice Nations; and his cousin Vickie Morris.
He is survived by: his wife Ellen Hughes Slayton (Wall); his children, Joshua Lewis Slayton (Rachel) and Kristin Nicole Slayton (Jim Simmons); his grandchildren, Riley and Kallie Slayton; his sisters, Karen Malcott (Bill) and Dianne Henson; and his siblings' children, Chris Nations, Ashley Kennedy, Hunter Malcott and Jordan (Malcott) Beck (Alan).
In lieu of flowers, the family respectfully suggests memorial donations be given to a fund benefiting David Lewis Slayton's memorial. These gifts can be made payable directly to McCullough Funeral Home with "David Lewis Slayton Memorial Fund" written in the memo line.
