He was predeceased by: his parents, Western Milton, Sr. and Johnnie Mae Smithwick; and his sister Diane Smithwick. He is survived by: his wife Karen Smithwick; his children, Shannon (Tami Smithwick), Angie (Brian Heinlein), Johnny Smithwick, Jr. and Jennifer (Jason Bryant); his grandchildren, Kaitlyn, Devin, Olivia, Chloe, Will, Addison and Bailey; and his brothers, Western (Rebecca Smithwick) and Gerald (Pamela Smithwick).
