He was predeceased by: his parents, Charles Monroe and Mary Morris Cook; his siblings, Gene Cook, Sherry Cook Cameron and Melvin Cook; and his pets, Wolf, Lassie and Angel.

He is survived by: his wife Ruby Weaver Cook; his children, Randy Cook (Jennifer) and Crystal Mitchell (Wayne); his stepchildren, Barbara Matheny and Jerry Weaver (Leigh); and his sister Loretta Cook Strickland. He is also survived by eleven grandchildren, eight great grandchildren and eleven nieces, nephews.
