He is survived by: his wife Cindy; his sister-in-law Anna Crouse (David); his niece Mina; his brother John Ballard (Joni); his nephew Jamie; his brother David Ballard (Rachel); and his nieces, Caroline, Olivia and Anna. He is also survived by many extended family, and friends.
