He was predeceased by: his parents, Herbert Earl and Lena Mae Smith Willingham; his wife Myrtice "Myrt" Barrentine Willingham; his son Earl Dwain Willingham, Sr.; his son-in-law D. Ray Thomas; his sisters, Melba Hayes and Bernice Martin; and his grandson Michael William Smith.
He is survived by: his daughters, Pamela Cook (Kenneth) of Perry and Lynn Smith (Melvin) of Lizella; his daughter-in-law Frances Willingham of Acworth; his grandchildren, Angela Dubois, Mike Thomas, Dwain Thomas, Tracy Thomas, Michelle Bily, Carl Willingham II, Earl Willingham, Jr. and Melvin Smith II; and his close friend Effie Cast. He is also survived by 16 great grandchildren; 10 great-great grandchildren.
