He was predeceased by: his parents, Clifford Blackwell and Hazel Cox Blackwell Belflower. He is survived by: his wife Teresa Blackwell of Warwick; his children, Johnny Blackwell, Jr. of Greenville, SC and Skye Baker of Idabel, OK; his sister Joann Jonson of Warwick; his stepsiblings, David Bellflower of Wisconsin, Andrew Bellflower (Dori) of Elko and Denise (Tim Davidson) of Columbus; and his grandchildren, Allison Blackwell, Caitlyn Jolly, Landon Mills, Leanna Martinez and Dixie Webb.

In accordance with COVID-19 requirements, face masks or face coverings are suggested and seating will be limited because of social distancing.
