He is survived by: his daughters, Melinda Lee Dale of Jacksonville, FL and Layla Ann Dale of Glen St; his grandson John Herman Lee, IV of Jacksonville, FL; his sisters, Cendy Pittman and Wendy Rivera; and his aunts and uncles, Ricky Mullaly (Vanessa) of Jacksonville, FL, Judy Pittman, Shirley New (Ronnie) of Jacksonville, FL and Lawrence Russo of Arkansas. He is also survived by numerous nieces and nephews.
