He was predeceased by : his father David Smith. He is survived by : his mother Wanda Smith; and his siblings, Susan Hannam (Ray), Phil (Cindy) and Matt (Jane). He is also survived by many nieces and nephews.

In lieu of flowers, the family requests that donations be made to the A.A. DSM Central Office to honor his memory and support others facing similar struggles.
