He is survived by: his wife Kathy; his daughters, Kelle Cook and Chris Duncan; his grandchildren, Nicole Cook (Nate), Garrett Cook (Katelyn), Becca Duncan and Brooke Antol (John); his great grandchildren, Elias, Edison, Elliott, Anthony, Cora and Callum; his niece Melissa Smith Plagge; his great-nephew Andrew; and his great-niece Cassandra. He is also survived by several other nieces, nephews and in-laws.
