He was predeceased by: his parents, Clifford Cook and Anita Cook (Freml); his son Cletus Jr. Cletus; his father-in-law Dale Bernholtz; his siblings, Bonnie Lavaas, Russ Cook and Gene Cook (Linda); and his siblings-in-law, Tim Solt and Debbie Heath.
He is survived by: his wife Jane Bernholtz of Carroll; his sons, Cory Cook (Jessica) of Polk City and Casey Cook (Caitlin) of Carroll; his grandchildren, Cameron, Cohen, Collins, Ivy, Leo and Mya; his mother-in-law Mary Bernholtz of Carroll; his sister Donna Solt of Fayetteville, NC; and his siblings-in-law, Vicki Jung (Jeff) of Sioux City, Joni Vallier (Greg) of Council Bluffs, Jill Brandenburg (Roger) of Omaha, Dan Bernholtz (Shari) of Sioux City, Steve Bernholtz of Des Moines and Jeff Bernholtz (Carol) of Estherville. He is also survived by many aunts, uncles, nieces, nephews, and cousins.
In lieu of flowers, memorials are suggested for a later designation.
