He was predeceased by: his parents, Gerald Putnam and Mary Putnam (Baier); his in-laws, Lyle and Emma Moeller; his brother Don; and his brothers-in-law, Larry Sorensen and Jon Moeller.

He is survived by: his wife Jessica; his daughter Jenifer Dvorak (John Katz); his grandchildren, Wolfgang (Olivia), William and Jessalyn Dvorak; his sister Rosie Sorensen; and his siblings-in-law, Fran Putnam, Jo Arbuckle, Joanne Moeller and Jay Moeller (Debbie Anne). He is also survived by nieces and nephews, cousins and friends.
