He is survived by: his wife Mildred Havlik; his children, Teresa Stolba, Brett Havlik (Elma), Patrick Sullivan (Brenda) and Molly Rausch (Ron); his grandchildren, Ashley (Brian), Kaitlin (Tony), Jared, Justin (Meredith), Cameron, Caroline, Nick (Harmany), Corryn, Melissa, Chloe, Claire, Charlotte and Crosby; his great grandchildren, Hadley and Nathaniel; and his sister Sally Gudenkauf (Ray). He is also survived by many nieces and nephews.

In recognition of his contributions to the sport, Doug was inducted into the Cedar Rapids Softball Hall of Fame.
