She was predeceased by: her father Jean Davis Burns; her great grandchild Bryten Becker; and her brothers-in-law, John Eaton and Ron Rohr. She is survived by: her husband Dennis Smith; her children, Kellie Baker (Clint Carruthers), Scott Baker (Tami), Jennifer Crozier (Brian) and Julie Johnson (Tim); her grandchildren, Shelby, Emily, Jessica, Libby, Sam, Bailey, Kelsey, Cael and Janie; her great grandchildren, Nora, Mia, Boone, Lennox, Bendyn and Blakelyn; her sisters, Sandy Besco (Clayton), Nancy Eaton and Jo Hunter (Terry); and her sisters-in-law, Bonnie Rohr and Shari Van Luchene (Joseph). She is also survived by many nieces, nephews, relatives, and friends.

Memorials may be made to the family and used to purchase a memorial bench for Phyllis that will be placed at the Lucas County Courthouse.
