He is survived by: his mother Lorraine Chizek (Young); his wife Cyndi Chizek; his children, Kaitlyn Halfpap (Levi) of Des Moines, Michael Chizek (Alyssa) of Des Moines and Joshua Chizek (Haley) of Des Moines; his grandchildren, Beau, Tate Chizek and Hayes; and his siblings, Carolyn Kilborn (Steve), Judy Brozik (Jim), Gary Chizek (Sheryl), Mary Manness (Mark), Ray Chizek (Paula) and Lori Mullins (Brent). He is also survived by numerous nieces, nephews, aunts, uncles, and cousins.
