He was predeceased by: his parents, Donald Azeltine and Dorothy Azeltine (Sullivan); and his sister Pamela. He is survived by: his wife Julie Azeltine (Baker); his children, Kelly Lackore (Jason) and Chadd Azeltine; and his grandchildren, Avery Lackore, Kaiden Lackore, Nathan Azeltine, Charlene Azeltine and Julie Azeltine. He is also survived by many nieces and nephews.

In lieu of flowers, memorials may be directed to Faith Lutheran Church.
