He was predeceased by: his father Clifford Robert. He is survived by: his mother May Lewis, Jr. (McMullen) of Clinton; his sister Heidi McMullen of Dubuque; his half-siblings, Torrie Lewis and Cody Lewis; and his uncle Norman Prock (Julie Parker) of Clinton. He is also survived by 3 nieces.
