He was predeceased by: his parents, Scott Oliver Duckworth and Norma Jean Duckworth (Swartz); and his brother Robert "Tony" Lee (Crystal Heyveld) of Creston. He is survived by: his children, Dani Marie Lisk (Lisk) and Michael Duane Duckworth, Jr. (Mike) of Creston; his wife Jo Anne Duckworth (McGuire); his children, Witmer (Kenneth) of Des Moines and Jason Duane Duckworth of Van Wert; and his sister Leesa Lester (Allen) of Creston. He is also survived by six grandchildren; five great-grandchildren.

In lieu of flowers, memorials can be directed to the family or the Creston Lions Club.
