He was predeceased by: his father Eugene Ortiz; and his grandsons, Jesse Lee James and Frank Junior Claybrook II. He is survived by: his mother Rachel Ortiz (Jones) of St. George, Utah; his spouse Shelly Stacey (Franklin); his daughter Cassandra Ortiz (Jonakio) of Davenport; his grandson Jonakio "Nocky" Williams; his siblings, Chuck Ortiz of Moline, Stephen Ortiz (Lia) of Fort Myers, Florida, Gregg of Coal Valley, Eric Ortiz of St. George, Utah., Darla Glidwell (Ted) of East Moline and Pamala Ortiz of St. George, Utah.; and his nephew Stephen Ortiz, Jr. of Moline. He is also survived by several nieces, nephews, and cousins.
