She was predeceased by : her parents, William Jirak and Alice Jirak (Chapman); her husband David Phillips; her mother-in-law Erma Swatek; her brother Clifford Jirak; and her brothers-in-law, Larry Phillips, Marlyn Siewert and Roger Hollenbeck.

She is survived by : her siblings, Steven Jirak (Cheryl), Leathia Siewert and Rita Hollenbeck; and her siblings-in-law, Cindy Blue (Jim), Roger Phillips (Marsha), Raymond Swatek, Danny Jo Swatek (Connie) and Jimmy Dale Swatrk (Pam).
