She was predeceased by: her parents, James Formaro (Jim) and Shirley Formaro (Swisher); her brother John; and her daughter Jeanne Davis. She is survived by: her son Joseph (Joey); her grandchildren, Kaylee, Emma, Elle, Toby, Chloe, Ean, Elizabeth, Maci and Jameson; her children, Johnny Davis (Johnny), Joey Davis (Holly), Toby Herrera (Isabella) and Maria Herrera; and her siblings, LuAnn Gilbert, Carmella Rose Sankey and Jimmy Formaro.
