He was predeceased by: his parents, Charles Forman and Evelyn Forman. He is survived by: his wife Arla Ralston; his sons, Cyrus and Isaac Forman (Kate); his granddaughters, Nora and Vera; and his siblings, William Forman (Christine Jones) and Nancy Forman. He is also survived by two nieces and one nephew.
