He was predeceased by: his parents, Louie Formaro and Margarite Formaro; his sister Mary Lou; and his brothers-in-law, George Clemons, George Harris and James Harris.
He is survived by: his wife Karen; his daughter Kim; his son-in-law Chris; his grandchildren, Thomas Francis "Tommy", Cortney, Joseph Louis "Joey" and Morgan; his great grandchild Addison James "Addie"; and his friend Dowling Lunch Bunch. He is also survived by neighbors, and relatives.
In lieu of flowers, a memorial fund has been established by the family to be specified at a later date.
