He was predeceased by: his stepfather John Hollingsworth; and his brother Kevin Martindale. He is survived by: his father Charles Martindale (Kathy) of Lewiston, Illinois; his children, Danielle Martindale of Donnellson, Iowa and Michael Martindale of Donnellson, Iowa; his grandchildren, Tavius, Serenity, Freya and Aurora; his mother Donna Hollingsworth (Fred Wilson) of Donnellson, Iowa; and his siblings, Richard Martindale of Donnellson, Jay Pritchard (Heather) of West Point, Iowa, Charlene Martindale (Markley), Caitlin Martindale of Lewiston and Celsey Martindale (Young). He is also survived by several nieces, nephews and cousins.

A memorial has been established in his memory.
