He is survived by: his parents, Jerome "Jerry" Jacobs and Lois Jacobs (Paulsen) of Elkader, Iowa; his wife Cheryl Sullivan; his children, Caitlyn (Nashua Lansing) of Elkader, Iowa and Jason Sullivan (Haley); his grandchildren, Elsa, Jory and Judd of Lost Nation, Iowa; his son Jeremy Sullivan (Megan); his grandchildren, Colt and Winston of Preston, Iowa; his siblings, Roger Jacobs of Mesa, Arizona and Dawn Janssen (Chris) of Gilmore City, Iowa; his mother-in-law Betty Geerts of Elkader, Iowa; his sisters-in-law, Catherine Hafsi of Cedar Rapids and Rebecca Corbisier (Chuck) of Preston, Iowa; and his godchildren, Mark Kroemer and Carly Janssen. He is also survived by several nieces, nephews, cousins.
