He was predeceased by: his daughter Joann Otten of Peoria, IL; his parents, Archie Cook and Myrtle Cook; and his siblings, Jean Beer, Jim Cook, June Glaser, Jerry Cook and Jack Cook.
He is survived by: his wife Helen; and his sisters, Jane DeLa Hunt, Joan Doster and Judy O'Leary.
