He was predeceased by: his parent Jerry Lee; and his grandmother Bette Herbert of Kahoka. He is survived by: his mother Linda Carol Herbert (Cattron) of West Burlington; his brother Jamie Herbert (Cheryl) of Cedar Rapids, Iowa; his niece Arya Herbert; and his stepsiblings, Mike Mitchell of Keosauqua, Iowa and Jill Holt of Georgetown, Illinois. He is also survived by aunts and cousins.
