He was predeceased by: his grandmother Eleanor Cook; his aunt Paula Cook; his grandparents, Walter Cook, Darlene Schantz and Carlton Schantz; his aunt Marilyn Whitney; his uncle Donald Schantz; and his pet Ono.
He is survived by: his mother Linda "Reanna" Cook; his sister Rachel Chrystal (Adam); his nephew Briggs Chrystal; his aunt Joanie Mills (Marc); his uncles, Martin Cook (Carolyn), Larry Schantz (Linda) and Dean Schantz (Kit); his cousins, Charles, Christen, Jana and Hayley; his friends, Aaron Reid, Scott Edgerly, Eric Nemmers and Jacob Pratt; and his brother Paul (Kim). He is also survived by caregivers; and biological father.
A celebration of life will be held on Sunday, October 18th 2026 from 3:00 PM to 7:00 PM at the Hiawatha Community Center (101 Emmons St, Hiawatha, IA 52233).
