He was predeceased by : his grandparents, Ed Folken, Earnest and Mary Parker; and his uncle Craig Parker. He is survived by : his parents, Gary Folken of Monticello and Christina Van Amburg; his children, JD, Jeremy and Natilie Folken; his stepfather Randy Van Amburg (Krista Robertson) of Center Junction; his siblings, Ashley Gartland, Gregg Van Amburg and Kurt Van Amburg; his grandmother Rose Mary Folken; and also, Emily, Max, Katie and Sara Derby.

In lieu of flowers, memorials may be made to Animal Welfare Friends in Monticello.
