He is survived by: his mother Carla Diercks; his stepfather Mark Diercks; his uncles, Mike (Leanne Theobald), John (Terry Theobald), Jim (Paulette Theobald) and Chuck (Roberta Smith); his aunt Mary Theobald; and his extended family, Gary, Karen Diercks, Jennifer, Jim Lofgren, Jake and Casey Diercks.
In lieu of flowers, memorials may be made to his mother, Carla, in Jay's honor.
