She was predeceased by : her parents, William Hemert and Thelma Van Hemert; her sons, Terry Alan and Craig David; her husband John; and her son Randy.
She is survived by : her sons, Randall Jay and John William; her family, Angela Remer, Katen and Ana Phyllis; her brother Dale; and her sister-in-law Arlene Van Hemert. She is also survived by parents-in-law, brothers and sisters-in-law.
Memorials will be made to the family and will be shared with Phyllis church and favorite local charities.
