He was predeceased by: his parents, Susie Mae Countryman (Bainter) and Donald Countryman; and his brother Delbert "Duff" Lawrence Countryman. He is survived by: his wife Kara Countryman; his children, Susan, Lacasta, Dave II, Adrianna (Walt), Elijah and Alexander; and his grandchildren, Eva, Eliana and Eli.
Duff's decision to donate his body to medical education inspired David to do the same, and David chose to donate his body to the Des Moines University Body Donor Program.
