He is survived by: his wife Patty Forkenbrock of Solon; his children, Ryan Holt (Erik), Jamison Forkenbrock (Corinne), Noelle Kostecki (Rick) and Nicolle Reyna (Gilbert); and his brother Jim Forkenbrock. He is also survived by 5 grandchildren.
