He was predeceased by: his uncle Cork Harmelink; his grandparents, Judy Wilson and Frasher; and his great-grandparents, Marvin, Helen Petersen, Eldon and Terri Kopel.

He is survived by: his parents, Heather DuBois (Jared) and Jeff Streuli (Jenn); his wife Chelsea Streuli (Forry) of Emmetsburg; his sons, Kash and Kacyn; his parents-in-law, Lori Forry and Paul Forry; his siblings, Kaelob Streuli, John Christopherson (Lauren) and Erin Forry (Sam); his grandparents, Debbie, Dan Kopel and Roger DuBois (Gwen); his aunts, Missy Roland (Dave), Julie Dwyer (Dave) and Laura Gilman (Chad); his uncle Brian Palmer; his cousins, Tanner Orwig, Alexis Roland, Jayme Currans and Josh Harmelink (Dani); and his nieces and nephews, Leighla, Aubree, Aaliyah, Hayden and Blaine. He is also survived by friends and brothers.

In lieu of flowers, a memorial fund has been established in memory of Trevor for his wife and boys.
