She was predeceased by : her parents, Robert Smith and Audrey Smith; her brother Jack Smith; and her husband Mike Slyby. She is survived by : her children, Lisa Carlson (Luke) of Ankeny and Jon Cox of Des Moines; her grandchildren, Courtney Strother (Robert) and Drew Carlson (Cathlin); her great grandchildren, Carter Carlson and Rory Strother; and her sister Gloria Jahnel (Bill). She is also survived by nieces and nephews.
Memorial contributions may be made in Carol's honor to Grace United Methodist Church of Des Moines.
