He was predeceased by: his parents, Frank Kulhavy and Ila Belle Kulhavy (Gallaher); and his brother Butch. He is survived by: his wife Sandy Coulthard; and his siblings, Ann Rouse and Dodd Kulhavy of Roper, NC. He is also survived by several nieces and nephews.

A memorial fund has been established.
