He is survived by: his wife Barb of Washington, Iowa; his children, Paul Horak (Lynn) of Washington, Iowa, Mary Pat Hawley (Dan) of Mequon, Wisconsin, Sarah Moses (Jim) of Springfield, Missouri, David Horak (Stephanie) of Urbandale, Iowa and Luke Horak (Suzanne) of Washington, Iowa.; his grandchildren, Mary Kate, Owen, Claire, Ian, Erin, Grace, Mackenzie, Gabrielle, Madalyn, Casaundra, Garrett, Eleanor, Brennan, Evan, Drew, Lauren and Emma; and his brother Tom Horak (Marsha) of Washington, Iowa.. He is also survived by seven great-grandchildren.
Memorials have been established for Hospice of Washington County and St. James Catholic School Foundation.
