He was predeceased by: his parents, Victor A. Dvorak and Helen Dvorak (Bartos); his son David Dvorak; and his sisters, Helen Powell and Joy Buchan.
He is survived by: his wife Rita Dvorak of Davenport; his children, Susan McMahon (John) and Doug Dvorak, MD (Lynne) of Littleton, CO; his brothers, Vic Dvorak of Hayfield, MN and Ed Dvorak (Sue) of Guttenberg, IA; and his grandchildren, Casey Dvorak, John Dvorak (Amy), Alex Dvorak, Amanda Beelow (Samm), Emily McMahon and Katie McMahon. He is also survived by 2 great grandchildren.
