He was predeceased by: his father Mark Oliver Leavitt; and his brother Steve Leavitt. He is survived by: his mother Linda Harris Leavitt of Blackfoot; his daughters, Brooklyn Leavitt and Annisten Leavitt; his siblings, Mike (Erica), Allen (Lisa), Rick, Kevin (Tiffany), Scott (Heither), Kristine (Jim), Todd (Tammy), David (Shannon), Jana (Grandon) and Brian; and his close friend Judd. He is also survived by nieces and nephews.
