He was predeceased by: his mother Shirley Blake Rinehart; his stepfather George Rinehart; and his cousin Kevin Blake. He is survived by: his father Curtiss Newell Blake Sr.; his stepmother Linda Blake; his sister Jamie Moeckli (Mark); and his stepsiblings, Karen Hughes (Jeff), Allen Gamel (Debbie), Curt Gamel (Mary), Dawn Elam (Dave) and Mike Rinehart. He is also survived by numerous Aunts, Uncles, Nieces, Nephews and Cousins.
