He was predeceased by : his significant other Marie Jackson; and his parents, Arta Mae Okelberry Slack and Wayne Thomas Slack. He is survived by : his siblings, Russell Slack of Sugar City, Daniel Slack (Lee) of Jerome, Alan Slack (Susan) of Grand Junction, Colorado, Katherine McEwen (Curtis) of Hazelton, Doug Slack of Jerome and Timothy Slack (Holly) of Sugar City; and his children, Lauren and Jeremy of Castro Valley. He is also survived by his daughters; many nieces and nephews, as well as several great-nieces and great-nephews.
