He was predeceased by : his parents, Clyde Dame and Maxine Dame. He is survived by : his wife Patricia Crandall (Patty); his child Leslie; his caregiver Brent; his sons, Mitchell Dame (Kari) and Nicholas Dame (Madi); and his siblings, Leslie Dame, Sherrie Lee (Mike) and Larry Dame (Vickey). He is also survived by many nieces, nephews and a great niece.

In lieu of flowers the family is requesting donations to St. Luke's Twin Falls Hospice.
