He was predeceased by: his parents, Rulon Mark Blake and Ethel Marie Armstrong Blake; his son Dayle Blake; and his siblings, Rulon Dayle Blake, Venice Blake Buell and James Robert Blake.
He is survived by: his former wife Betty Jean Moulton; his children, Hal Mark Blake (Iola), William Blake (Dayle), Sandy Brown, Robyn Nickum, Jana Kolbet (John) and Curtis Gardner (Karla); and his wife Dot. He is also survived by 10 grandchildren and 17 great grandchildren.
In lieu of flowers, the family suggests donations be made to Primary Children's Hospital.
