He was predeceased by: his parents, Robert H. Lewis and Patricia Ann Crouch Lewis; and his grandmother Margaret Crouch. He is survived by: his wife Kathleen; his children, Danielle Enno of Pocatello, ID, Matthew Lewis of Mapleton, UT and Kaitlin Ouzts of Chubbuck, ID; and his siblings, Roberta Knight of Anaheim, CA and Timothy Patrick Lewis of Anaheim, CA. He is also survived by seven grandchildren; and five great-grandchildren.
