He was predeceased by: his parents, Otto Ernest Johns and Margaret Elizabeth Jensen; and his child Chandra (Darrik). He is survived by: his sisters, Margaret, Virginia, Colleen and Carolyn; his spouse Candis Winward; his children, Wade (Ami), Cary (Bill), Paula (Bill), Kay Lyn and Jack Andrew (Saundra); his stepson Brooks (Kim); and his children, Brady (Kristyn), Chris (Tiffany) and Tyson (Sara). He is also survived by many grandchildren and great grandchildren.
The family expresses their gratitude to Autumn Care in Hyde Park, and Aegis Hospice Care for their care of Jack as he finished his battle with Alzheimer's disease.
