He is survived by: his wife Laurie Denison; his children, Daniel Cook (Bonnie), Rachel Haslhofer (Gerald), Suzanne Skogen (Jeffrey), Jenny MacTaggart (Joshua), Carrie Bonner-Cook (Linzee), Camille Dixon (Nathaniel) and Heather Cook; his nephew Sean Walsh; his niece Rachel Polasek (Robert); and his siblings, Brent Cook, Andrew ElRay Stewart-Cook, Lucy Hicks, Karl Cook, Sara Cook, James Cook and Ellen Helm. He is also survived by 20 grandchildren.
