Johnson (Connie) of Rotonda West, FL, Jeff Johnson (Pat) of Mendon, OH, Steve Johnson (Brenda) of Fernandina Beach, FL, Janelle Sites (John) of Rockford, OH, Lyndell Smith (Bill) of Ohio City, OH, Kim Smith (Ed) of Mendon, OH. and Terry; and his son Adam Robert Johnson.

He is survived by : his wife Lori Vicchy; and his children, Paige and Jake Johnson.
