He was predeceased by: his parents, David Cook and Helen Cook (Adams); his wife Frances Cook; his siblings, Neomia Means and Robert Allan Cook; and his nephews, Robert D. Hill and Jeff Hill.
He is survived by: his children, David Cook, Jr. (Janet), Scott Cook and Amanda Fowler (Chris); and his sister Mabel Ann Hill. He is also survived by seven grandchildren; fourteen great-grandchildren; two great-great grandchildren.
