He was predeceased by: his parents, Erwin B. Forde and Betty L. Forde. He is survived by: his wife Amy Forde; his sons, Ethan W. Forde, Eric D. Forde (Makayla) and Trey Kuzmanov; his sister Carla Gude; his stepmother Roslyn J. Forde; and his friends, Amy, Dan and Boston Marathon. He is also survived by relatives and friends.
Visitation was held on Wednesday, September 16th 2026 from 3:00 PM to 8:00 PM at the Beidelman-Kunsch Funeral Homes & Crematory - South Naperville (24021 Royal Worlington Dr, Naperville, IL 60564).
