He was predeceased by: his parents, Stephen Banasiak and Rose Banasiak; his sons, Michael Banasiak and David Banasiak; and his brother Jerome Banasiak.
He is survived by: his wife Ruth Banasiak; his children, Susan Young (John) and Timothy Banasiak (Mary); his granddaughters, Rachel Young (Max Barnett) and Katherine Banasiak (David Murphy); and his siblings, Anthony "Tony" Banasiak (Pat) and Loretta Gomulka (late Frank).
